PSI appoints Risk Review Group
The PSI has appointed a Risk Review Group to advise on the issues around the recently reported error in patient underdosing with the seasonal influenza vaccine in some pharmacies. The Group will look at what the learnings might be from this issue.

PSI statement and report on pharmacy vaccinations issue
The PSI has published the report it furnished to the Department of Health and the HSE earlier today regarding the issues around the underdosing of some patients with the flu vaccine in pharmacies.

Advice for public on concerns over pharmacy vaccinations
Pharmacies are now working to identify all the patients who got the lower dose and all of these patients will be contacted by their pharmacist over the coming days to come back to the pharmacy to get revaccinated.
